Externship Evaluations

All externships require evaluations from the host organization and the student. The evaluator at the host organization should be the individual identified as the supervisor on the "Externship Information Form" previously submitted.

*Note: For Public and Corporate Externships done in the National Aquarium, use the Aquatic Medicine/Aquaculture form.

Student Evaluation of the Experience

ALL private practice externships should be evaluated using the Private Practice (Student) form (Word).

All P/C, Pathology and Research externships should be evaluated using the Center for Public & Corporate Veterinary Medicine, Student Evaluation of Experience form (Word).

Submission

Both evaluation forms are due in Academic Affairs not later than noon on the first Thursday following the completion of the externship.

Both evaluations must be submitted for the student to receive credit for the externship. It is the sole responsibility of the student to insure that the forms are properly submitted. In the event that an organization establishes a pattern of failing to submit evaluations in a timely manner they will be removed from the list of acceptable hosts.
